Your call preparation checklist
Choose what the desk actually told you. Leave an answer open if you did not ask or the answer was unclear. This tool never converts your selections into a verified hospital badge.
Read out the district, address and source facility ID. Ask which entrance or registration desk to use.
Answer: Not asked / unclearA listing captured earlier is a starting point. Ask about the status today and where the current official entry can be checked.
Answer: Not asked / unclearAsk for the package name and code for the procedure your clinician proposed, and whether this branch is approved for it. A specialty code is not a package approval.
Answer: Not asked / unclearAsk which tests, medicines, implants and follow-up services apply to the proposed package. Request an explanation of any proposed payment before agreeing.
Answer: Not asked / unclearAsk whom to meet, when to arrive, which documents the official desk requires and whether the entrance meets your accessibility needs.
Answer: Not asked / unclearCall date: Not recorded
6 of 6 preparation questions still need an answer. An unanswered question does not mean the hospital is ineligible.
Next: Did the desk match the exact branch and facility ID?
Your answers stay in this open page’s memory. They are not saved to your account, browser storage or our database. Reloading or leaving clears them; use Print / Save as PDF if you want a copy. Do not enter patient names, Aadhaar or card numbers, or medical details. This page loads no ads, Analytics or Clarity.
What to do when answers disagree
If the hospital says it no longer accepts the scheme but an older list still includes it, keep both statements separate. Check the list’s date and ask the official help channel for current branch status. Do not assume that an old contact still reaches the Ayushman desk. Ask the person answering to identify the desk’s role or direct you to the current contact.
If the branch is listed but cannot confirm your proposed package, your next step is a package-specific answer. General approval for a hospital or specialty does not settle the procedure, authorization or services included. Take the procedure advised by your clinician to the desk; this checklist does not select a treatment or assess whether it is appropriate.
If proposed charges are unclear, ask for the service and reason in writing. Keep any records privately for an official enquiry. You do not need to send them to this directory. Read how to prepare a PM-JAY service enquiry or complaint.
Use the plan at the right time
Prepare these questions before a planned visit, then reconfirm if the appointment, branch or proposed treatment changes. An answered checklist records a conversation at one time; it does not replace the hospital’s official checks or establish eligibility. In an urgent situation, seek appropriate emergency care instead of waiting to complete an online worksheet.
If a number fails or an address is wrong, use “Report a data issue” on the hospital profile and include a public official source if available. Corrections enter a review queue; a visitor’s checklist answers never update a hospital’s public status. Missing data is a reason to ask a question, not evidence that the hospital closed or was delisted.
Official channels and further reading
NHA HEM hospital search is the official hospital directory to consult. If the portal cannot be reached, that failure does not establish the status of any hospital. PM-JAY’s published beneficiary-rights document identifies 14555 for service-delivery grievances; use official channels for eligibility and service decisions.
