1. Find a hospital
Search by name or PMJAY ID. Choose a state, then a district, then a known city or locality. If the city is missing, use district search; many source lists do not provide a city.
2. Read the evidence
“Listed in source” means the hospital appears in the linked official list. The source date tells you when that evidence applied. The catalog update date tells you when we imported it. Neither guarantees current empanelment.
3. Confirm with the Ayushman desk
Ask about current empanelment, your eligibility, the treatment package and documents you need before arranging your visit. This directory does not make eligibility or treatment decisions.

Official source snapshots are retained with dates, identifiers and row references. An explicit delisting or suspension hides a hospital from default results. A missing record triggers review. It does not, by itself, prove delisting.
