How to Read a PMJAY Hospital Record: IDs, Dates and Contacts

A useful hospital record should help you identify the right branch and see what still needs confirmation. These worked examples turn the source fields into practical decisions, without treating a listing as an admission guarantee.

Published by Arsh Kapoor · Editorial preparation guide · updated 18 October 2026. This explains evidence and questions to ask; it is not medical advice.

Start with branch identity

Copy the full facility identifier exactly as the source supplies it. Our imported evidence includes identifiers with different formats; a value does not become invalid simply because it lacks a HOSP prefix. A website’s internal record ID is a separate identifier and must not be presented as an official facility ID.

For example, the Lucknow directory retains the Institute of Medical Sciences and the similarly named Ram Manohar Lohia Combined Hospital as separate records. A similar name or area is insufficient evidence to merge them. Use the identifier, full institution name and address together. See the institute’s branch-specific contact references.

What to ask

“Please confirm the full facility name and address for this ID. Is this the branch handling my planned visit?”

Three dates describe three different events

The following is an illustrative timeline, not a new observation about any hospital.

How to interpret dates without making old evidence look new
FieldExampleWhat it establishes
Source as of3 August 2026The authority supplied this date for its list or decision.
Captured27 September 2026The list was retrieved then. Its stated date remains August.
Imported1 October 2026The evidence entered the directory then. This is not a new status decision.

If no source date was supplied, keep that uncertainty visible. A refreshed webpage footer or recent download does not prove that an individual hospital row has changed. Check the row’s underlying source and ask the authority or branch for current confirmation.

Choose a contact by its role

In KGMU’s own directory, online OPD appointment help and the general PBX are different contacts. The number retained from a hospital list can serve another administrative role. Keeping both sources with labels is more useful than silently replacing one number.

KGMU’s official contact directory supports this distinction. Our KGMU profile separates these published roles; it does not claim they are dedicated Ayushman authorization lines.

SGPGIMS provides a different example: its homepage labels a public-relations counter 24×7, while a separate OPD schedule describes clinic arrangements. That counter label is not evidence that every clinic operates continuously. Check the institute’s own references, then ask which desk handles your appointment and package question.

What to ask

“Is this the appointment desk, general enquiry counter or Ayushman desk? Which team can confirm my next step?”

Keep status, ownership and package evidence separate

Government or private describes ownership. An accreditation field is a different claim. Neither determines whether a specific procedure is approved for a particular branch and patient. Our “listed in source” label describes a recorded appearance, rather than a fresh confirmation of acceptance.

Explicit suspension, de-empanelment or blacklisting evidence takes priority over a general ownership listing in our default discovery rules. Missing from a later download, a failed website request or an unanswered number is a reason to investigate; none establishes delisting by itself.

A Ministry of Health and Family Welfare 3 February 2026 update links readers to HEM for the detailed private hospital list. For the proposed procedure, use the package checklist and ask the hospital for a branch-specific answer.

Turn the evidence into a useful next step

  1. Open two candidate records in the comparison tool. Check the full names and locations first.
  2. For a missing contact, go to the official source or institution website. Do not borrow a number from a similar branch.
  3. For an undated listing, keep the retrieval date as retrieval evidence and ask for the latest explicit status.
  4. Use the private call planner to record unanswered questions and print a checklist. No account is required.
  5. If a field in our directory is wrong, submit the public evidence through that profile’s correction form. An official treatment complaint follows a different process.

The result should be a clear set of questions, not a score that pretends to assess clinical quality. Medical decisions and eligibility determinations remain with the relevant professionals and official services.

How these examples were prepared

Published by Arsh Kapoor. This is an editorial explanation of public evidence and directory behavior, prepared with AI assistance. It is not a clinical review, a claim of first-hand hospital experience or medical advice.

The institution contact pages were inspected for their stated contact roles; no telephone calls were made and no appointment, package or current empanelment was verified. Official contact pages were reviewed on 18 October 2026; an undated source remains undated. Read the evidence and corrections policy or send a correction with a public source.
